Although the heterogeneity of leukemic blasts in the acute transformation of chronic myelogenous leukemia(CML) has been well documented, promyelocytic crisis is extremely rare and investigations on the characteristics of this type of crisis remains insufficient. As the clinical feature, hematologic diathesis was frequently found since the promyelocytic crisis is associated with DIC. Many reported cases, cytogenetic analysis showed a t(15; 17) superimposed upon the Ph1 chromosome, but other additional chromosomal aberrations were also reported. We experienced a case of a 28-year-old male whose diagnosis was CML with additional chromosomal abnormality, developed an promyelocytic crisis 5weeks after diagnosis. In this case cytogenetic study revealed 50, XY, double Ph1, +4, +8, +19.
